Since you see "refuse" underlined, and "they refuse" sounds fine, you may just want to go to the answer choices.
First, you see a classic 2-3 split among the answer choices: 2 use "they" and 3 use "the clinic" or "it." Which is better? Since the first part of the sentence refers to the clinic, the second part should as well, so keep the choices with "the clinic" or "it" and eliminate (1) and (3). You have two options for the verb: is it correct that the clinic "refuse to attend" or "refuses to attend"? Refuses is correct, so eliminate Choice (2). Finally, look for a difference between (4) and (5). Does the clinic refuse to "attend to" a pet or "attend for" a pet? Attend to is the way we'd say it, so eliminate Choice (4).
